Medical – 3 options (Aetna)
1 – HMO option – HNO 0/70 Colorado (70%, no deductible, $30/$50 copays)
1 – HSA option – HDHP 5000 Plus (80% with $5k deductible)
1 – PPO option – Flex 1000 Plus (80% with $1K deductible, $25/$50 copays)
0 day waiting period
The HSA option is covered 100% by ADVODA for the employee and dependent costs, with annual employer contribution of $900 for employee or $1500 for employee+ spouse or dependents.
Dental - 4 options (MetLife)
Preventative, Core, Enhanced and Enhanced Platinum – each plan covers preventative, basic, major and orthodontic services at different levels
Advoda pays 95% of employee benefit (with the exception of the Enhanced and Enhanced Platinum plan), 47% of dependent benefit
Vision - 1 option (MetLife)
1 plan with in and out-of-network benefits
Advoda pays 100% of employee benefit, 50% of dependent benefit
Flexible Spending Account (Section 125) available for medical and dependent care
Short Term Disability / Long Term Disability Policies
Covers approx 60% of earned wages tax free, $10K monthly maximum benefit
ADVODA pays 100% of benefit, however the amount paid by ADVODA is included as wages on each paycheck. For the benefit to be paid post tax, the premium has to be taxed.
Life Insurance / ADD
· 1x Salary (based on W2 wages) per employee paid 100% by ADVODA
401(k) Plan with 4% company match (Mass Mutual)
ADVODA will reimburse up to $150 combined for high speed internet at home and cell
phone/data plan (for email) charges as well as other work related expenses.

Athletic Club Membership Reimbursement
Advoda will reimburse membership costs for gym/athletic club membership for employees
up to a maximum set annually by the company. Currently the cap is $100.00 per employee,
per month. Must be employed for 6 months to participate.
Paid Time Off
Annual PTO Bank – based on tenure with the company, starting at 120 hours (15 days) per year, pro-rated based on start date
Holidays - 8 paid holidays (New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, Day after Thanksgiving, Christmas Eve, Christmas Day)
Food and Beverage Accommodation
Advoda has allotted a monthly budget to allow for breakfast food/snacks and water to be
available to employees in the kitchen on an unlimited basis (up to the budget allocated).
